YOUNG Carer Support Practitioner

Number of hours per week: 30

Salary: £21,402.86 pro rata (£24,970 FTE)

Type of contract: Permanent

Work base: Sanford House, Sanford Street, Swindon, SN1 1HE (with community and home visits)

Reporting to: Head of Service Delivery

ROLE Purpose

  • Support young carers aged 5 to 17 to thrive, achieve their potential, and enjoy the same opportunities as their peers.

  • Complete Young Carer Needs Assessments and develop personalised support plans tailored to individual needs.

  • Provide practical, emotional, and advocacy support to reduce the impact of caring responsibilities on education, health, and wellbeing.

  • Deliver one-to-one support, group activities, workshops, and holiday programmes.

  • Work collaboratively with families, schools, health professionals, and community organisations to coordinate effective support.

  • Promote a whole-family approach to ensure young carers and their families receive the right help at the right time.

KEY Responsibilities

  • Assess the needs of young carers in home, school, and community settings.

  • Develop and review personalised support plans that build resilience and improve outcomes.

  • Help young carers access education, leisure activities, social opportunities, and specialist support services.

  • Provide advocacy and liaise with schools and partner agencies to ensure young carers are understood and supported.

  • Deliver engaging groups, workshops, and activities that provide respite, social connection, and personal development opportunities.

  • Make referrals and coordinate support with services such as mental health, youth, respite, and family support providers.

  • Attend multi-agency meetings and contribute to Early Help and Team Around the Family plans.

  • Raise awareness of young carers and support professionals to identify and respond to their needs.

  • Maintain accurate records, assessments, and outcomes data.

  • Respond appropriately to safeguarding concerns and contribute to effective risk management.
