PARENT Carer Support Practitioner

Salary: £17,122.29 pro rata (£24,970 FTE)

Work base: Sanford House, Sanford Street, Swindon, SN1 1HE (with community and home visits)

Reporting to: Head of Service Delivery

ROLE Purpose

  • Provide personalised, strengths-based support to unpaid carers across Swindon, with a particular focus on Parent Carers.

  • Complete statutory Carers Needs Assessments in line with the Care Act 2014 and Children and Families Act 2014.

  • Develop tailored support, wellbeing, and contingency plans to help carers achieve their goals.

  • Offer information, advice, emotional support, and income maximisation guidance.

  • Manage Emergency Card Scheme plans to ensure carers have support in place during unexpected situations.

  • Deliver ongoing casework, early intervention, and crisis support when needed.

  • Work within a multi-skilled team to provide seamless, holistic support for carers and their families.

KEY Responsibilities

  • Conduct person-centred assessments using guided conversations to identify strengths, needs, risks, and desired outcomes.

  • Create and review personalised support plans that reflect whole-family circumstances.

  • Support Parent Carers in navigating services such as SEND, Early Help, and Team Around the Child (TAC).

  • Manage a varied caseload, prioritising support based on individual need.

  • Respond to crisis situations and provide practical support to prevent escalation.

  • Maintain and review Emergency Card Scheme arrangements.

  • Signpost carers to specialist services and community resources where appropriate.

  • Build strong partnerships with local authority teams, health services, education providers, and voluntary sector organisations.

  • Attend and contribute to multi-agency meetings to improve outcomes for carers and families.

  • Maintain accurate records and support contract monitoring, reporting, and service evaluation.

  • Identify and respond appropriately to safeguarding concerns involving adults and children.
