ADULT Carer Support Practitioner
Number of hours per week: 25
Salary: £17,835.71 pro rata (£24,970 FTE)
Type of contract: Permanent
Work base: Sanford House, Sanford Street, Swindon, SN1 1HE (with community and home visits)
Reporting to: Head of Service Delivery
ROLE Purpose
Provide personalised, holistic and strengths-based support to adult carers across Swindon.
Complete statutory Carers Needs Assessments in line with the Care Act 2014.
Develop tailored support plans and contingency plans to meet carers' individual needs.
Offer information, advice, emotional support and practical guidance.
Assist carers with income maximisation and access to benefits.
Manage and review Emergency Card Scheme plans.
Deliver ongoing casework, crisis intervention and wellbeing support.
Work collaboratively with Swindon Borough Council, health services, education providers and community organisations.
Contribute to positive outcomes for carers while meeting contractual and quality standards.
Operate flexibly within a multi-skilled team to provide seamless support.
KEY Responsibilities
Complete Carers Needs Assessments and develop personalised support plans.
Provide information, advice, emotional support and practical guidance to carers.
Support carers to maximise income and access relevant benefits.
Manage and review Emergency Card Scheme plans.
Respond to crises and deliver preventative, wellbeing-focused support.
Work closely with statutory, health and community partners to improve outcomes for carers.
Maintain accurate records and contribute to service monitoring and reporting.
Identify and respond appropriately to safeguarding concerns.
Participate in duty, triage and service-wide support activities.
Engage in ongoing training, supervision and professional development.
